All students entering a Florida school for the first time must have proof of immunization to enroll. |

| School Health Services Immunization Clinic | ||||||||
|
School Health Services, located at the Family Service Center on the campus of Keller Intermediate School, provides an Immunization Clinic for children on Thursday afternoons from 1:00 – 4:00. Parents should call 259-6551 to schedule an appointment. |

| Physical Examinations | ||||||||
|
Florida law requires that first time entrants into public schools, including kindergarten or any other grade, have a physical exam within 12 months prior to enrollment. If a student has ever been in a Florida school, a new physical is not required. A copy of this Florida physical examination must be made available to the school. |

| Sports Physicals | ||||||||
|
Students participating in athletics at Baker County High School and Baker County Middle School must have a current physical to participate. School Health Services coordinates a day during the summer for athletes to receive a sports physical for the coming year. |
