The Baker County School Board and the Baker County Health Department collaborates to provide a registered nurse at each school site under the direction of the Baker County School Board. Each school clinic offers:

  • First aid, evaluation, referral of injuries and illness
  • State and locally mandated health screenings such as vision, hearing, scoliosis, height & weight and blood pressure
  • Classroom instruction on current health issues
  • Identify student with health problems, follow-up, referral and home visits when necessary
  • Administration of medication's with written parental permission
  • Provision of medical procedures as ordered by a physician (catheterizations, breathing treatments, etc.)

Immunization Requirements

7th Grade

Students entering 7th grade are required to have received the following immunizations:
  • Hepatitis B Series
  • Second dose of Measles Vaccine (preferably MMR)
  • Second dose of Measles Vaccine (preferable MMR)
  • Tetanus-Diphtheria Booster (Td)
   

Parents are urged to begin these shots early.

       
    Kindergarten          
Immunizations required for kindergarten entry are:
   
  • Diphtheria-Tetanus-Pertussis Series
  • Hepatitis B Series
  • Measles-Mumps-Rubella (two doses, preferably MMR)
  • Polio Series
  • Varicella
       
   

All students entering a Florida school for the first time must have proof of immunization to enroll.

     
               
    School Health Services Immunization Clinic      
   

School Health Services, located at the Family Service Center on the campus of Keller Intermediate School, provides an Immunization Clinic for children on Thursday afternoons from 1:00 – 4:00.  Parents should call 259-6551 to schedule an appointment.

     
               
    Physical Examinations        
   

Florida law requires that first time entrants into public schools, including kindergarten or any other grade, have a physical exam within 12 months prior to enrollment.  If a student has ever been in a Florida school, a new physical is not required. A copy of this Florida physical examination must be made available to the school.

     
               
    Sports Physicals        
   

Students participating in athletics at Baker County High School and Baker County Middle School must have a current physical to participate.  School Health Services coordinates a day during the summer for athletes to receive a sports physical for the coming year.
